Thank you for completing your client paperwork!

If this is your first appointment, please note that there should be two separate links in your email. One for a registration and one for an assessment.  Both of these will need to be completed prior to your first appointment.

How to join your appointment?

If your appointment is virtual then you will be able to join our HIPPA compliant video conferencing system powered by ZOOM by going to our virtual waiting room. From there you will scroll until you see the name of the clinician that you are scheduled to see. If you do not already have ZOOM downloaded on your phone or computer it will download. Once your clinician is available for your session, your clinician will greet you.

If you are coming into the office for your first appointment, you can find the step by step directions from your door to our office by clicking on the hyperlinked text on Our Locations page. When you arrive at the office you will want to wait in the waiting room for your clinician to come out and bring you back to their office.

First Appointment Checklist :

  • Provide credit card to keep on file for automatic bill pay

  • Email a copy of the clients insurance card (front and back)

  • Complete both the "Client Assessment" and "Client Registration" that were emailed to me
