After you have scheduled your first appointment, the below forms can be completed online. Once you click submit, they will be emailed to us directly.
Your New Client Registration and Assessment will be emailed to the email address that was provided during intake. The email will have two separate links for you to click and complete. Once submitted these are delivered directly into your electronic client chart in our HIPAA compliant system. If you have not seen this email, contact our front office and they can send these again.
